I have been asked to participate in a research study that investigates how post-graduate relationships are developed, established and maintained. This study is important because there is little research regarding relationship development after college. Results from this study may aid in understanding how the dating world outside of college is evolving. This survey will take approximately 15 minutes to complete. The results from this study will be used to write a paper that will fulfill a course requirement, as well as possible presentations at a professional meeting and/or publication.

I understand that in participating in this study I will retain absolute anonymity from all parties, even the researchers, that there are no questions that can identify me personally, and that my participation is completely confidential. I also understand that completion of the survey is voluntary and that I may stop participating in this study at any time for any reason. I understand that data collected for this study may be published.
I understand that:
A. There are no foreseeable risks associated with completing the following survey.
B. There is no direct benefit to participating in this study and I will not be compensated for my time.
C. Any questions I have concerning my participating in this study will be answered by the above Principal Investigator, who can be reached at the contact information at the top of this page.
D. I may refuse to participate or may withdraw from this study at any time without negative consequences. Also, the investigator may stop the study at any time.
E. If I have any questions, comments or concerns about the study or the informed consent process, I may write or call the Office of Research & Sponsored Programs Administration/Institutional Review Board, Chapman University, One University Drive, Orange, CA 92866; Telephone (714) 628-7392 or (714) 628-2805.
F. I acknowledge that I have a received a copy of this form and the research Participant’s Bill of Rights

SOCIAL and BEHAVIORAL RESEARCH



Any person who is requested to consent to participate as a subject in a research survey, test, or experiment has the following rights:



1. To be told what the study is attempting to discover.



2. To be told about any potential risks or discomforts of the things that may happen to him/her during the research activity.



3. To be told if he/she can expect any benefit from participating and, if so, what the benefits might be.



4. To be told what other choices he/she has to participate in research (i.e., additional non-research options for extra credit, etc.)



5. To be allowed to ask any questions concerning the study both before agreeing to be involved and during the course of the study.



6. To be told the extent, if any, to which confidentiality of records identifying the subject will be maintained.



7. To refuse to participate at all before or after the study is started without any penalty or loss of benefits to which the subject is otherwise entitled.



8. To receive a copy of the signed and dated consent form.



9. To be free of pressures when considering whether he/she wishes to agree to be in the study.
