This survey has been created by the Childhood Bereavement Network – the hub for all the organisations and individuals working to support bereaved children and their families across the UK.

We are preparing a campaign to increase awareness of the importance for all parents to have plans in place in the event that they die before their children have grown up. We would like to get the views of anyone - parents, professionals, the public - who might have ideas about how parents can prepare. We know it is difficult to think of this possibility but we would really appreciate your helping us understand how parents can do this. You do not need to be a parent yourself to answer these questions.

We know that it can be upsetting to think of this. If you don't want to answer any question, please just skip to the next one, and you are free to stop doing the survey at any point. The survey is anonymous although theree is a section where you can share a story if you would like.
