Consent and Eligibility
We would like to invite you to participate in a web-based online survey that will help us to better understand the barriers that people face when trying to access dental care. This is a research project being conducted by the Florida Institute for Health Innovation (FIHI).
The survey should take approximately 10 minutes to complete.
Your participation in this survey is voluntary. You may refuse to take part in the research or exit the survey at any time without penalty. You are free to skip any question if you do not wish to answer for any reason. Your responses to this survey will be anonymous.
At the end of the survey you will be asked if you are interested in joining a more in-depth conversation with other parents and FIHI about this topic. If you choose to provide contact information such as your phone number or email address, your survey responses may no longer be anonymous to FIHI, but we will not share your responses with others. No one's name or identifying information will be included in any publications or presentations based on this information, and your responses to this survey will remain confidential.
