1. Introduction

This survey aims to evaluate the attitudes of people with motor neurone disease (MND/ALS) about end of life issues such advance decisions, assisted suicide and euthanasia, with additional questions about low mood and social circumstances. Such issues have recently received considerable media attention. Lord Joffe’s Assisted Dying for the Terminally Ill Bill was defeated on its second reading in the House of Lords on 12 May 2006 by a margin of 148 votes to 100.

This survey has been designed by Barts and The London Hospital NHS Trust. It can be answered by anyone with motor neurone disease or by relatives/carers on behalf of people with motor neurone disease.

It is a quick, anonymous online survey. You do not have to answer all of the questions and you can stop at any time (see top right).

The purpose of this survey is to provide evidence that will inform the public debate and lead to better patient education and management in the future. We would very much appreciate your contribution!
