MH User Survey

This survey is being undertaken to help Coventry City Council and Coventry and Rugby Clinical Commissioning Group to assess the quality of the Mental Health services that are currently provided.

Please complete this survey if you live in Coventry or if you are a patient of a GP with a practice in either Coventry or Rugby.

The survey is based on the questions used by the Care Quality Commission when they inspect services. This questionnaire can be used to provide your views on any of the services that you use.

If you wish to give views on more than one service please complete a separate questionnaire for each service so we can clearly identify views relevant to each service.

The information from this survey will be used to improve our services, the information may be shared with partners to improve service delivery across the city.
