Question Title

* 1. Listening to Patients is a project run by the Faculty of Education and Health at The University of Greenwich as an independent consultation. It takes around 20 minutes to complete and aims to seed innovative approaches to healthcare through listening to patients' voices, patients' narratives and patients' experiences and focuses specifically on the relationship between the patient and the professional. CONSENT
• I have read the information about this evaluation.
• I have received a contact email to ask questions and discuss the evaluation if I so wish.
• I understand the survey is an evaluation of my experience being a patient for the purpose of generating collaborative understandings of about best practice and will be used to inform an evaluation for use in clinical practice, to support medical education and to seed research collaborations inclusive of patients' voiced experiences.
• I understand all information will be anonymised except where I have chosen otherwise and that this is clearly indicated in the relevant questions.
• I understand that I am free to withdraw from this study at any time without giving a reason for withdrawing (if I am, or intend to become, a student at the University of Greenwich) without affecting my future with the University without affecting any medical or nursing care I may be receiving.
• I understand that my consent is voluntary.
