IFT15 Student Monitor Application

 

Question Title

* 1. As a student monitor, we consider you to be an important member of our staff and a representative of our organization. For a better understanding of what is expected of you as a volunteer, please read the student monitor rules and duties and sign the acknowledgement at the bottom of the online application form.
By serving as a monitor, IFT will provide you:
• Complimentary registration to IFT15
• Complimentary lodging in a quad-occupancy shared room during the meeting (4 day and 3 night stay)

IFT asks that you agree to:
• View the On-Demand Training
• Attend a mandatory onsite training to be held in Chicago, IL on July 11 at 3:00p.m. CST. 
• Serve as a room monitor up to six (6), 1½ hour IFT scientific and applied sessions including but not limited to: technical, keynote, supplier solution, poster sessions, Awards Celebration, etc. as assigned by IFT.
• Dress appropriately for all sessions. Dress code for all session monitors is business casual (No jeans, shorts, or sneakers.)
• Conduct yourself in a professional manner.
• Always be on time for all scheduled sessions.
• Adhere to all rules and duties. Students may be required to reimburse IFT for registration and lodging fees if they fail to adhere to the rules and duties set forth by IFT.

*Note - if you are receiving funding from any source IFT source, you are ineligible to participate as a Student Monitor.

By completing this survey, I am hereby applying to serve as a Student Monitor for IFT15 in Chicago, IL July 11-14, 2015 and, acknowledge that I have read and agree to the Student Monitor Duties and Rules.
