Why we have created this mechanism
HQIP receives a great deal of constructive anecdotal feedback regarding national clinical audits – both positive and covering areas people feel could be strengthened.

There is always room for improvement with any project and therefore HQIP has developed this mechanism to collect information, identifying common themes and ensure messages are delivered to the most appropriate organisation, e.g. national audit providers, National Advisory Group on Clinical Audit and Enquiries (NAGCAE), the Department of Health etc. If good practice is to be shared and common concerns addressed and/or rectified, your feedback is essential. The system has been devised to enable you to feed back specific comments about specific audits and is open to anybody participating in any aspect of national clinical audits.

How the feedback mechanism works
This resource has been designed to allow for continuous use and will be analysed by HQIP on a monthly basis.
In addition to this feedback to appropriate organisations, a quarterly report will be posted on the HQIP website. This report will include detailed analysis of comments received; how HQIP has used this information and, over time, the actions taken by providers, contractors and other organisations.

What the system SHOULD be used for
1. Highlighting examples of good practice within national clinical audit
2. Making constructive suggestions for improvement to projects within the Quality Accounts list
3. Highlighting ongoing or unresolved issues.

What the system should NOT be used for
1. Technical issues in relation to projects (these should be addressed to the relevant provider)
2. Projects with national coverage which do not appear in the Quality Accounts list.

What we will do with your feedback
1. To project providers:

     For NCAPOP audits:
     a. As part of existing feedback and monitoring arrangements for current contracts
     b. In discussions as part of development of new projects and contracts

     For non-NCAPOP audits:
     Comments will be fed back to NAGCAE/DH

     For both types:
     Decisions on further inclusion in Quality Accounts.

2. To the clinical audit community:
Quarterly reports of themes and actions will be published on the HQIP website.

How

You can also provide one set of general comments by following the options as you progress.
