Title of Study: Acculturation, Generational Status and Help Seeking Attitudes

Introduction: You are invited to participate in a research project being conducted by Keila Roman, M.A, Regent University and Dr. Vickey Maclin. The purpose of this form is to give you information that may affect your decision whether to say yes or no to participation in this research.

Purpose: The purpose of this research project is to gain a better understanding of individuals from various ethnic backgrounds and generational status attitudes towards seeking help.

Procedures: Participants will complete this study online following this informed consent. Participants will first complete a series of questions regarding generational status to determine eligibility to participate in the study. Participants will then complete two questionnaires and a demographics questionnaire. We anticipate the study will take 15 minutes to complete.

Risks and Discomforts: There are no foreseen risks from participating in the study. There is some possibility that you may be subject to risks that have not yet been identified. Benefits: By participating in this study you will be contributing to our understanding of help seeking attitudes in individuals from various generational status and levels of acculturation.

Right to Refuse or Withdraw: Participation in this study is voluntary and you may refuse to participate or discontinue answering questions at any time without penalization.

Anonymous and Confidential Data Collection: Data collection will be entirely anonymous and will remain secure through the online survey database.

Confidentiality of Records: All responses to measures will be confidential and will not be traceable back to the individual. Any published results will not contain personally identifiable information and your name will not be associated to any of the data.
