This year, the online form requests the majority of the mandatory questions previously asked as part of the Ballot form. We have included this information ahead of time to expedite the process and to help ensure your eligibility to submit a nomination. The information must be answered in full in order to download the appropriate category-specific ballot form.
The ballots themselves have been created into a PDF form that is downloadable and can be saved to anyone’s local drive so they are able to forward amongst their team members for review prior to submitting.
To qualify for an award, all mandatory items below must be answered. Persons nominated must be employed as an indeterminate, term, or equivalent employee in the respective level of government (including persons employed in the military, RCMP, crown corporations, boards and agencies) at the time of their nomination.
ELIGIBILITY: In all three groups (Federal, Provincial and Municipal), awards will be given for initiatives, projects or accomplishments that realized tangible/quantifiable results (with supporting endorsement contact information or documentation) during the period March 31st, 2013 to April 1, 2014. Submissions related to initiatives or projects of all sizes and any scope are encouraged. The submitted project/pilot/initiative must be completed in the timeline outlined above, with supporting endorsement contact information or documentation, otherwise the submission will not be considered.
