There are 3 Clinical Commissioning Groups (CCGs) in South Nottinghamshire – Nottingham North and East, Nottingham West and Rushcliffe. They commission health services to be provided to patients across South Nottinghamshire – Broxtowe, Gedling, Hucknall and Rushcliffe.

Nottingham University Hospitals NHS Trust (NUH) is a large provider of acute health services in South Nottinghamshire, through the Queen's Medical Centre, City Hospital and Ropewalk House.

NUH and the 3 South Nottinghamshire CCGs want to get your thoughts on their services.

Your thoughts are valuable to us. All collected responses will help NUH and the 3 South Nottinghamshire CCGs to examine our current service provision and to determine what areas we need to improve on to enable improved services to patients.

Answer each question from your own personal experience, either as a Patient; Parent; Guardian or Carer.

For each question, please answer on a scale of 1 to 4.
1 = Not at all; 2 = Sometimes; 3= Most of the time; 4 = Always

We have used this scoring method as it matches the grading process used for the Equality Delivery Scheme (EDS2) which is the system used by the NHS to embed equality and diversity. In using this scoring method we can ensure your responses are included in this grading process.

This questionnaire also includes the Friends and Family Test question. Please complete this should you have used NUH in the last 12 months. This question does not fall under the 1 to 4 scoring criteria.
