Welcome to the WFMC Patient Survey!
Thank you for taking part in this survey to help us improve the care we provide for you and your family as part of the Whole Family Medical Care community. We are exploring the possibility of starting a direct primary care practice (DPC) so that we can offer our proactive patients more access and time with their provider.
Direct primary care is a membership program in which patients pay a monthly fee for health care services instead of paying a copayment at every visit and billing insurance. It is becoming more popular in several regions of the country. The monthly membership fee would replace medical bills with a single payment for all health care services provided by WFMC during the month.
The membership fee would provide:
1. Office visits, including the option of scheduling longer appointments with Dr. Host and your health care team.
2. Communication with Dr. Host outside of scheduled appointments (for example, by phone, through secure messaging).
3. Support for healthy lifestyle choices (for example, counselling, weight loss, acupuncture, massage therapy, life coaching).
4. Discounts for lab work, prescription medications, and other health services provided outside of WFMC.
