We are requesting your feedback, name and organization to improve our services and supports for you. Your input will only be used in combination with the responses of others participating in the survey, unless you give specific permission to share it publicly, in the form of a testimonial.

We will only use your information for the purposes of communicating with you on relevant matters. We will not share your information with any third party except to the extent they are processing information on behalf of The Wheel and where we have an agreement in place with that third party processor.
