I am a graduate student at the UMDNJ School of Public Health working in conjunction with the NJ State Department of Health on a project to develop a new comprehensive educational material for pregnant women. This survey contains no identifying information and your responses cannot be traced back you in any way. By choosing the 'I Agree' option and clicking the 'NEXT' button at the bottom of this page, you are implying your consent to participate in this survey. This will allow your responses to be included in the study and the development of a new material. If you do not wish to participate, please click the 'X' at the top of this webpage to close the survey page.


Your responses in this survey, in its entirety or any one question, are completely voluntary. There is no penalty if you decide not to participate in the survey and you may stop the survey at any time. This survey does not address sensitive or threatening subject matter and your responses will be used only for the development of a new educational material. Please understand that although no method of transmission over the Internet is perfectly secure, we have taken steps to help protect your confidentiality. The survey will not collect any identifying information such as your name, email address or IP address. In addition, we are using an SSL encrypted version of SurveyMonkey.com which is one of the most trusted versions of internet security also used by banking and government websites.
