Health Insurance Rate Review Survey
Thanks for taking the time to complete this survey from the Arizona Department of Insurance (ADOI).

The ADOI wants to make health insurance rate information submitted to us by health insurance companies more readily available to consumers. We also want to help the public better understand our procedures and authority to review health insurance company rates. This survey will inform us about what health insurance rate information would be valuable to you!
Please note: This survey is NOT collecting information about the following types of health insurance: Medicare Advantage, Medicare supplement, AHCCCS plans, long term care, HMO’s, or large employer plans (50+ group). These survey questions relate ONLY to major medical health insurance for individuals and small employers (less than 50).
The ADOI is conducting this survey as part of a grant received from the U.S. Department of Health and Human Services. The total grant award is $1,000,000; however, spending is limited to $550,441.00 which is the amount in the budget ADOI submitted with its grant application.
