2015 UHN Community Needs Assessment

Thank you for your interest in completing and assisting in the gathering of the United Houma Nation's 2015 Community Needs Assessment.  This survey is intended to gather the most recent data and demographics of currently enrolled tribal citizens.  All enrolled tribal citizens over the age of 18 are encouraged to complete this survey.  All responses are confidential and eligible respondents will have the opportunity to sign up for a $200 drawing as a thank you for your participation.  All entries must be completed by Monday, June 1st to be eligible for the drawing.
