How to Respond to this Survey

This survey is primarily to learn about problems using Marketplace Health Plans. However, we know that you may also be hearing about problems with Medicaid, and so we have created a question at the end that allows you to report problems using Medicaid.

This survey IS NOT about problems with enrollment, but is limited to problems using the plans. If the problem is that the person has applied for Medicaid, but has not yet received a notice of eligibility, they may call UHCAN Ohio's Help Line at 614-456-0060 x 233 for additional information about accessing Medicaid.
