IOWA COMMUNITY ACTION AGENCIES - CLIENT NEEDS ASSESSMENT
Iowa's community action agencies are conducting a study of the needs individuals and families may be experiencing in their lives. Results from the study will be considered by the community action agencies for planning, developing, and delivering agency programs, services, and activities.
INSTRUCTIONS:
There are 32 questions. Almost all of the questions are multiple choice. Please answer each question by checking the appropriate box (or boxes) or typing a response. If a question does not apply to you, please leave it blank. The survey takes about 10 minutes to complete.
