Thank you for participating in the 2012 TreePhilly Yard Tree Giveaways! We are asking for your help by providing feedback to improve our program. This questionnaire will only take about 5 minutes to complete. Please have the person who was most involved in interacting with TreePhilly staff and caring for your tree fill out the survey. And, if you fill out the survey by January 1st, you will be entered into a raffle to win a free watering bag for your new tree ($25 value)!
