Thank you for filling out this survey. We're interested in your feedback about the use of the SACS which is incorporated into the PHO patient information system. The SACS is a valid, NZ designed, youth appropriate screening instrument which has been tested in Kiwi young people. Hopefully it will enhance engagement with young people in your practice, provide better information about their difficulties and promote appropriate referrals to AOD services.
