Thank you for your interest in Working Well in New Jersey, a free worksite wellness toolkit provided by the New Jersey Department of Health (NJDOH).
In order to determine the impact of this toolkit on the health and wellness of employees in New Jersey, NJDOH would like to collect a few details about your worksite now, and again in a few months after your worksite has had a chance to implement some of the strategies in the toolkit. Please fill out the following brief 15-minute survey.
Upon submission of the survey, a PDF of the toolkit will be emailed to you. In a few months, NJDOH will send you another brief survey. Your responses to both surveys are very much appreciated, as they will be instrumental in understanding how Working Well in New Jersey has contributed to improving employee health and wellness.
