Before you start please read the following which explains why we are running this survey and how your feedback is important to us.

Each year, each GP practice is given a fixed budget by the Department of Health to pay for different types of health care. We do not have the resource to do everything the practice or our patients would like us to, but your views are an essential part in helping us to decide what is most important to our patients, and where we should prioritise any resource available.

As a patient of this practice, we want to know what you think about the services we provide for you.

We are asking for your views on the following:

• How easy it is for you to find out information about your GP practice
• If this information meets your needs
• How you think we could reduce waiting times
• Your ideas for improving the system for making appointments
• What you think about our premises
• How satisfied you are with our services, overall
• How you would like to be involved in planning changes to improve services in the future.

This short survey contains 23 questions, most of which are to be answered by ticking either one box or a number of boxes, or by rating your satisfaction on a scale.

For some questions, where we think it is important we hear what you want to say in your own words, we have provided a box for you to write in.

This is your chance to tell us what you really think about your local GP practice and we hope you will take this opportunity of giving us your feedback.

Your participation in this survey is entirely voluntary and you have the right not to participate or withdraw your participation at any time.

The information you share with us will be treated in the strictest confidence and will not be made available to any third party.
