TPREF Pharmacist-Provided Diabetes and Hypertension Care Program |
Program Overview
The Tennessee Pharmacists Research and Education Foundation (TPREF), in partnership with the Centers for Disease Control and Prevention (CDC) and the Tennessee Department of Health (TDH), is working to increase the role of pharmacists as non-physician practitioners and providers of care within the state of Tennessee.This grant program will provide the TPREF with the ability to credential and train 100 pharmacists in diabetes and hypertension prevention and management, to support the provision of pharmacist-provided patient care services to 100 patients, and to accredit 100 practice sites through the American Association of Diabetes Educators (AADE) Diabetes Education Accreditation Program.
TPREF is actively seeking interested pharmacists to apply to participate in this grant program. Participating pharmacists and pharmacies will be selected based on criteria, including targeted medically underserved areas, as defined by the CDC and TDH and included in the grant narrative. This grant program is limited to pharmacists practicing in the state of Tennessee. The deadline to register for this program for 2017-2018, the last year of the grant, will be July 31, 2017.
