Title: Teacher’s Perceptions of the Likelihood of Implementation, Feasibility, and Perceived Obstacles Regarding Classroom based Psychological Recommendations

Purpose of the Study:
This is a study in psychology that is being conducted by faculty and doctoral students at Fielding Graduate University. The purpose of this study is to examine psychological recommendations for a student given from a psychological report.
Procedures:
You will be complete an informed consent; please print a copy of this Informed Consent for your records. Then, you will be asked to complete a 30-minute survey. The survey includes questions about yourself and your professional teaching experience (e.g., Number of years of teaching experience, Race/Ethnicity, etc.). After answering demographic questions, you will read a brief summary from a psychological report. You will then be asked to answer questions regarding the recommendations, potential obstacles, and overall function of the report (e.g., How likely are you to implement a recommendation, etc.).
Participant Selection:
You have been invited to participate in this study because you are a teacher. The survey will take 30 minutes to complete, which includes reading the informed consent and completion of the survey in full.
Confidentiality:
All responses will be confidential as allowed by the communication or delivery method used. We will NOT know your IP address when you respond to the Internet survey. We will ask you to include your e-mail address when you complete the Internet survey so that we can include you in the drawing for the gift certificate. However, your email address will not be stored with data from your survey. Instead, you will be assigned a participant number and only the participant number will appear with your survey responses. Only the researchers will see your individual survey responses. The list of e-mail addresses of our participants will be stored electronically in a password-protected folder; a hard copy will be stored in a locked filing cabinet. At the completion of the study, if you would like to get a summary of the aggregate results of the study, please contact one of the researchers at the number or email listed below and a summary will be provided to you.
Benefits of this Study:
You will be contributing to knowledge about classroom recommendations frequently used in psychological reports. At the completion of the study, if you would like to get a summary of the aggregate results of the study, please contact one of the researchers at the number or email listed below and a summary will be provided to you.
Risks or discomforts:
No risks or discomforts are anticipated from taking part in this study. If you feel uncomfortable with a question, you can skip that question or withdraw from the study altogether. If you decide to quit at any time before you have finished the questionnaire, your answers will NOT be recorded.
Alternatives:
There are no alternatives to participation in this study. The choice is to participate or to not participate.
