If your concern relates to an actual incident involving a patient and you are a NLSHD member of staff then report this incident via the IMMS notification system.
If you are a patient or relative then bring this to the attention of the ward Nurse Unit Manager or the Patient Representative
Where your concern relates to a patients likelihood of potential medication error, then discuss your concerns with the medical team looking after the patient as well as the ward NUM and ward pharmacist.
Where your concern relates to a general observation of medication safety practices on the ward, please notify the ward NUM and make a submission via this link.
Where there is a high likelihood of immediate risk please escalate your concerns by telephone or in person to the facility pharmacy department also.
