Dear school representative,

The WA Health Promoting Schools Association Inc. (WAHPSA) is a not for profit, non-government entity advocating a whole school approach to health and wellbeing. Members of WAHPSA include Foodbank WA, Cancer Council WA, KidSafe WA, Community Development Foundation, WA School Canteen Association and Sexual and Reproductive Health Western Australia and other representatives from the health and education sectors.

WAHPSA is seeking information from Western Australian schools to identify areas of improvement in school health and to ascertain how WAHPSA can best support schools in promoting health and wellbeing to students, teachers, parents and the wider community. WAHPSA will use the information to develop resources and support to meet identified needs. More information on WAHPSA is available at www.wahpsa.org.au

This survey relates to preventive health and wellbeing in the school setting, rather than specific medical, occupational health and safety, and student behaviour matters (for example anaphylaxis management or behaviour management).

This survey will take approximately 10 minutes to complete. Participation in the survey is voluntary. Participants are at liberty to withdraw at any time without prejudice or negative consequences; non-participation will not affect an individual’s rights/access to WAHPSA services.

All information obtained from this survey will be kept confidential and the data produced will be de-identified. At the conclusion of the study a summary report will be publicised on the WAHPSA website. No participants will be identifiable in this published material.
