Valley Health CHNA Survey

You have been asked to participate in this survey because Valley Health System (Winchester Medical Center, Warren Memorial Hospital, Shenandoah Memorial Hospital, Page Memorial Hospital, Hampshire Memorial Hospital, and War Memorial Hospital) wants to identify the unique health-related concerns of people living in each hospital's community. Your answers will help your local hospital respond to needs in your community. There are no right or wrong answers to these questions. We know that this information is personal.
