Paediatric Video Telemetry Admission Questionnaire
This questionnaire has a maximum of 18 questions and should be completed each time there is a child admitted for non invasive VT.
Thankyou for taking the time to fill in this questionnaire. This is part of a nationwide audit of the paediatric video-telemtery units. We are interested in gathering data from all of the paediatric VT units regarding the indication for VT, patient demographics, affect on management and adverse events of patients undergoing VT.
We would be grateful if you could complete this survey each time a patient is admitted for VT. This could be done immediately following the admission or on a monthly basis. We aim to collect admission data over a 12 month period.
