The Institute for Public Health Innovation (IPHI), with funding from the National Network of Public Health Institutes (NNPHI), is organizing a Virginia state forum to examine our understanding of population health and the role of community health workers (CHW) in bridging health care and population health in Virginia.

As a regional partner in the development, coordination, evaluation, and implementation of public health initiatives, IPHI is working collaboratively with the Virginia Department of Health and other partners in Virginia in implementing this first-ever VA state forum event. The one-day forum, tentatively planned for March 2015, will bring public health and health service providers in Virginia together to define population health and explore the role of CHWs in promoting population health and health equity within our health care systems and communities. It will also explore opportunities to expand the CHW workforce as we strive towards improving the overall health of our State.

As members of Virginia’s public health community, we are seeking your input on this Population Health and Community Health Workforce Survey in order to inform the planning of the state forum.
