Introduction
With so many changes taking place in our nation's health care system, NAMI, the National Alliance on Mental Illness, seeks to learn how your health insurance covers mental health and/or substance use care for you or anyone else in your life who may use these services.
This survey consists of questions about experiences with health insurance coverage. Please answer the questions to the best of your ability and feel free to say, "I don't know" if you come across difficult questions. You might find it helpful to refer to your insurance policy information, but it is not necessary. To navigate within the survey, use the 'next' and 'prev' buttons at the bottom of the page. The survey should take 15 to 20 minutes.
Your participation in this survey is confidential. The information from this survey will only be used in ways that do not reveal who you are. Your responses will be combined with all other responses and reported without identifying features.
