1. CONSENT FOR PARTICIPATION
I understand that this confidential study involves research that will be conducted by Susannah Robb Kondrath, B.A., a doctoral student in clinical psychology at the California School of Professional Psychology at Alliant International University (AIU) in Los Angeles, California, as part of a research project. I qualify to participate in this study because I am over the age of 18 and below the age of 65, live in the United States & am an American university student, speak English as my primary language, am not pregnant, do not have evidence of cognitive impairment, do not experience symptoms of active psychosis, do not have a diagnosis of PTSD or other major mental impairment, & am willing to participate in an online survey.

The purpose of this study is to investigate general life satisfaction of students. While this study will not benefit me directly, the data will contribute to scientific knowledge in the field of psychology. I understand that my participation in this study will involve the completion of one survey questionnaire. My involvement will take approximately 30 minutes.

My participation in this study is voluntary & I may refuse to participate or withdraw from this study without any consequences. My identity and responses on the questionnaires will be kept in strict confidence & will not be released to anyone without my separate written approval. All information about me will be protected to the limits allowed by law. I understand that the confidentiality of my information cannot be guaranteed if I indicate that I am a threat of harm to myself or someone else, or if I indicate that I have knowledge of a child, a dependent adult, or older adult (65 years or older) who is being abused or neglected. Additionally, it is possible that the data I provide could be subpoenaed or requested by the federal government under the Patriot Act (in which case I would not be able to be notified, but the principal investigator would bring an appeal to the Foreign Intelligence Surveillance Act (FISA) court on my behalf in order to maintain confidentiality). In these cases, whatever information is necessary to protect the safety of any parties at risk of harm will be disclosed, as mandated by law.

Only the principal investigator, Susannah Robb Kondrath, & project chairperson, Nicholas Noviello, Ph.D, will have access to the data collected. My name will never be associated with my responses. Non-identifiable raw data will be stored securely on computers at the research institution before being destroyed. Personal information will only be viewed by researchers directly involved in the study. Informed consent forms, raw data that contains identifying information, & other personal data will be retained for seven years following A the completion of the study; or B after a report of the work is published/publicly presented. Anonymized datasets (i.e., with all identifying information removed) or anonymous datasets (i.e., where I never provided identifying information) may be retained indefinitely. The researchers may wish to utilize these data in the future for purposes beyond the initial intent of the study (e.g., doctoral dissertation).

Answering of some questions may make me feel uncomfortable. If this occurs, or if I have any further questions, I may contact the principal investigator, Susannah Robb Kondrath skondrath@alliant.edu or Nicholas Noviello, Ph.D. at CSPP-LA at Alliant International University, 1000 South Fremont Ave, Unit 5, Alhambra, CA 91803. If I would like psychological assistance, the researchers will find assistance in my area within 24 hours & offer referrals. Psychological assistance will be provided to me at my own expense if I request it. If I experience an emergency due to or during the study, I will call 911 or go to the nearest emergency room.
