Dear participant,

If you are a healthcare professional, who work either full-time or part-time in hospitals, clinics, or private practices, you are invited to participate in a research study looking at healthcare professionals’ therapeutic commitments in working with patients with alcohol use disorder (AUD). Healthcare professionals have a crucial role in identifying, recognising, and providing appropriate treatments and referrals for patients with AUD. Identifying factors that influence healthcare professionals’ therapeutic commitments in working with patients with AUD may provide insight on appropriate support needed to help healthcare professionals in working with patients with AUD.

Participation will involve responding to a questionnaire that will take approximately 15-20 minutes. Minimal risk is associated with participation in this study; however, you may feel that some questions ask information that may be sensitive or personal. We do not foresee any possible risks from participation beyond those encountered in everyday life. In case of the unlikely event of distress, please contact your GP. Alternatively, you can contact Lifeline Australia (13 11 14 or https://www.lifeline.org.au/); Lifeline International (https://www.lifeline.org.au/About-Lifeline/Lifeline-International/About). Participation is COMPLETELY VOLUNTARY and you are free to exit the questionnaire at any stage without affecting your status now or in the future.

Your responses to the questionnaire are anonymous and confidential. You will not be identifiable in any way by your responses. The researcher will take every care to remove responses from any identifying material as early as possible. Likewise individuals’ responses will be kept confidential by the researcher and not be identified in the reporting of the research. However the researcher cannot guarantee the confidentiality or anonymity of material transferred by email or the internet. Data will be securely stored on University of South Australia server and in a locked cabinet at the research supervisor's office at University of South Australia, Magill Campus, C1-06, for a minimum period of 5 years.

Submission of the completed questionnaire will be considered your consent to participate. If you have any questions or concerns in relation to the study, please contact Jillian Dorrian, email: Jill.Dorrian@unisa.edu.au, tel: +61 8 830 24484; Vera Halim, email: halvy003@mymail.unisa.edu.au.
