1. Informed Consent Statement

Faculty and students at Illinois State University, Normal, Illinois, U.S.A., are conducting a research study to identify challenges in the development of map symbols for hazard, risk, and crisis/humanitarian relief mapping by humanitarian organisations, international agencies, national governments, and NGOs around the world. We invite you to participate in the study by completing the following short questionnaire.

The purpose of the questionnaire is to identify common challenges encountered by organisations and agencies in order to guide improved "best practices" for map symbology development to support humanitarian relief activities. In the following survey, you will be asked to provide input on your experiences in the use, design, and development of maps and map symbols used for hazard, risk, and crisis/humanitarian relief mapping activities. Please answer each question to the best of your ability. The survey is easy to complete. Your input will be of great value to the international community of humanitarian organisations and NGOs, and results from the project will be available upon request.
