Sunshine Health Implementation Survey - Regions 5 and 6

Sunshine Health has contracted with Sandata Technologies, LLC to implement the EVV Project in Florida. EVV is a telephone and computer-based system that documents the time care-givers begin and end delivering services to enrollees and will be used to submit home visit claims to Sunshine Health. The EVV system and the Sandata portal will be used to schedule visits and will be the only method to submit claims for services.

There is no cost for the EVV program to the enrollee or provider agency.

The EVV Project will be mandatory for all agencies that provide, and for all enrollees who receive the following services:
•Adult Companion Care
•Attendant Care
•Chore Services
•Homemaker Services
•Intermittent Skilled Nursing
•Nutritional Assessment and Risk Reduction
•Personal Care Services
•Respite Care
•Occupational Therapy
•Physical Therapy
•Respiratory Therapy
•Speech Therapy

To support the implementation of the Electronic Visit Verification (EVV) system, Sandata Technologies needs to confirm and collect some key information about your agency in order to set up your Santrax telephony EVV account.
This survey will take approximately 5-10 minutes to complete. Before you proceed, please have the following information available to complete this survey:
•Primary and Secondary Contact Information for your locations in Regions 5 and 6; address; phone; email
•Technical Person/Administrator in your office
•Number of Staff in your agency

You will also be answering questions about your agency’s ability to meet the systems requirements to run the web-based EVV program.
