Welcome to the 2015-2016 Sister Cecilia B. Abhold Award nomination process. Health Outreach Partners and Community Health Partners for Sustainability recognize that outreach programs across the nation work hard throughout the year to ensure that underserved communities receive quality health care services. This award will be presented to an organization with 330g, 330h, or 330i funding and an innovative outreach program. The nomination process allows us to get to know your program and understand the exemplary work you do to improve the lives of underserved communities.
The recipient of the award will be provided with the following benefits:
-A conference registration (selected from pre-approved list)
-Up to $400 travel stipend for conference
-Recognition at conference during a plenary session
-Lunch with HOP and/or CHPFS staff during the conference
