Thank you for registering for the 2014-15 Shuler Hensley Awards. This is the "SHORT FORM" / REGISTRATION FORM. THIS FORM IS DUE FRIDAY, SEPTEMBER 19, 2014.

The first 50 applicants will be accepted. All subsequent applicants will be added to the waiting list in order of receipt. Waiting list applicants may be admitted to the official participant list upon the withdrawal of a registered applicant.

Schools accepted into the program will be contacted via email on September 22, 2014. Visit www.shulerawards.org for program information.
