For completing this survey, you will be entered to win $400 towards a radon mitigation system for your home. To qualify for this drawing you must meet the following criteria:
1) Have received your Radon test kit through the Safe Car. Safe Home Program. You can contact the American Lung Association in Illinois at 217-787-5864 to find a location nearest you that is involved in our program.
2) You must have a finishing test date before September 30, 2014
3) Your test results are 4.0 pCi/L or greater
Winners will be notified within 2 weeks from contest ending date. Winners must provide a copy of their test results and at least one estimate from a licensed radon mitigation professional within two weeks of being contacted or another winner will be drawn. The radon mitigation system must be installed before the prize will be awarded. The prize will be awarded in one of the following options; 1) Mailed to winner after receiving proof of installation of a radon mitigation system in the home (copy of the installation bill and a picture of the system) 2) The American Lung Association will work with your chosen radon mitigation professional to pay them the $400 directly. If a winner does not have the radon mitigation system installed by January 31, 2015, they prize money will be forfeited.
