* 1. Title of Project: “Spiritual But Not Religious”
The purpose of this study is to document the beliefs and practices of people who identify as “spiritual but not religious,” as well as people who identify as religious but not spiritual, both religious and spiritual, and neither spiritual nor religious.

Name of Investigators:
The investigators for this project Steven Fouse and his instructor Professor John Schmalzbauer of the Religious Studies Department. The survey was designed by Steven Fouse, MA student in the Department of Religious Studies at Missouri State University.

Research Procedure:
You will be asked a series of qualitative (open-ended) and quantitative (multiple-choice) questions on the following topics: demographic information, religious history and current religious identification and practices, political and economic beliefs and affiliation.

Benefits/Risks:
There are no direct benefits to you for participating except for the value of preserving your thoughts for the historical record. There are no major risks to you if you choose to participate.

Voluntary Participation:
Participation in this project is voluntary. You may choose to withdraw from the project at any time by contacting Steven Fouse or Professor John Schmalzbauer at 417-836-5918 or Fouse519@live.missouristate.edu.

The goal of this project is to generate survey data for a student MA thesis. With your permission, we would like to use your survey results in all public presentations and publications associated with this project. In addition, we would like to deposit your survey results in Meyer Library at Missouri State University or some other suitable research archive. Scholars, journalists, writers, and the general public would have access to your survey results. Your name would NOT be attached to the survey results.
