School Safety Survey

Thank you for participating in our survey. We are asking each St. Luke student in your household to complete this survey.

This survey will help us evaluate our school environment and the impact of our Second Step program. Please answer honestly and openly as this survey is completely anonymous. Please leave names out of the survey, as we do not want any identifying information. We are requesting grade level for statistical purposes.

By completing this survey and printing the confirmation page you will be entitled to a free dress coupon. Simply write your name on the paper and send it in to the office.
