Note to Parents and Caregivers

This survey was prepared by parent, consumer, and advocacy groups concerned about recent changes in how California provides mental health support to children ages 3 to 22 in school. Your participation will help us advocate for students who need mental health support and services to benefit from their public education.

Your answers are confidential. You can complete this 10-question survey in under ten minutes. Please complete this survey by 11:00 p.m. next Wednesday, February 8.

"Mental health supports or services" includes one-on-one or group counseling; psychotherapy; prescription and evaluation of medications for your child by a psychiatrist; the dispensing of medication by a school nurse or other health aide; mental health evaluation for the purpose of making a diagnosis or determining a disability; residential treatment; or any other mental health supports and services identified in your child's IEP.
