Thank you for your interest in the Specialist Palliative Care and Advance Care Planning Advisory Services Project survey.

The survey is being conducted by a consortium of national health, academic and aged care organisations, including Palliative Care Australia, Respecting Patient Choices, CareSearch, the University of Queensland, Queensland University of Technology, the Australian and New Zealand Society of Palliative Medicine, Leading Age Services Australia and Aged and Community Services Australia.

It is part of a multi-faceted project funded by the Australian Government, which includes a dedicated national hotline for GPs and aged care providers to call for immediate access to specialist palliative care advice and information about advance care planning. The service will be free and available 24 hours a day, 365 days of the year, and complemented by a suite of web-based resources, with tablet and smart phone access, as well as training and linkage activities.

The online survey should take about 20 minutes to complete and the findings will help to ensure the advisory service resources are developed to meet the needs of your organisation.

In order to collect the most detailed information possible, it is preferable that the survey be completed at the local service manager level, and in one sitting to ensure answers are fully captured.
