Registration - Provider Initial/Reassessment Train the Trainer Class 2014

 
The Department of Health and Senior Services, Division of Senior and Disability Services, (DSDS) has made a decision to partner with Home and Community Based Services(HCBS)care providers to gather the necessary information in order for DSDS to determine the eligibility of participants for HCBS. The information is gathered by a face-to-face reassessment of the participant. DSDS provides the training to complete the process of reassessment. By completing this registration, you are agreeing to attend the training, taking the test and, if successful, training others or completing the reassessment process yourself.

1. In order for a provider to participate in the reassessment and care planning process, the provider must use qualified staff as trainers and assessors. All trainers and assessors must meet the same qualifications as an Adult Protective and Community Worker II.

All training is conducted at the Truman State Office Building, 301 West High Street, Jefferson City, MO. Attendees must attend the entire class to take the test at the end of the session unless they are there to retest.

An enrollment confirmation will be sent to the email address listed below.

At the conclusion of the course, an exam will be given. The attendee must successfully pass the test with a grade of eighty percent (80%) or better to be a trainer.
