Welcome to the first step in finding appropriate help for your teen
Congratulations on taking the difficult steps to find help for your teen. We understand that it is hard to know if a program is a good fit for an individual. Please allow us to partner with you in the process. The Project Patch Admissions team will proceed carefully to determine whether our program is appropriate for your teen and if not, help identify core areas to consider and guide you to other resources.
The purpose of this quick survey is to help identify some of the behaviors your teen is experiencing which will guide our Admissions staff as they work with you to help your teen.
Once we have reviewed the Pre-screen, our Admissions Department will be in contact within two business days. Our business office is open Mon - Thur 8 am-5 pm (PST) and Fri 8 am-3 pm (PST).
