INTRODUCTION
The Faculty of Occupational Medicine seeks to gain feedback on the revised SLE DOPS forms, following the pilot which began on 15 January 2014. The DOPS forms have been expanded to include audiometry, spirometry, biological monitoring, communication activity and a generic tool. The existing workplace form has also been revised.
We would be grateful if you could spare 5-10 minutes to complete the survey, giving details of any SLE DOPS assessments or your experience/observation since the introduction of this pilot. All responses will be anonymous and all feedback provided will be immensely helpful.
