We know many individuals have received many different types of care, but we are asking you to focus on a single one. Please choose from the 'highest' level of care you received and base your responses on that:
- Mother and Baby unit/Psychiatric ward with baby accompanying me (highest)
- Psychiatric ward without my baby
- Community/Home Treatment/psychiatric nurses or other mental health staff would visit me at home (lowest)
Not all questions will seem relevant (especially for those treated at home), but please complete as many as you can, we've tried to make it applicable to all.
