Welcome to Our Survey

This is the second of The Border Practice Patient Surveys. We have designed this survey to be quick to complete and share your views with us.

The questions start with the usual queries about you. This gives us an idea of the preferences of our various patient groups. We then go on to invite your thoughts on the main topics that were raised by patients in our previous survey. This will enable us to further work on our action plan.

We thank you for your time and input to our survey and wish you a very Happy Christmas and New Year.
