IMPORTANT DUE DATES:

THURSDAY, NOVEMBER 15, 5PM:
Submit the names of two Recommenders on the Letters of Recommendation Form

MONDAY,NOVEMBER 26, 5PM:
Submit the completed application form online.
Deliver an original transcript and a resume to Dr. Leanne Field's office in Painter 1.06.
YOU MUST MEET THE FOLLOWING ELIGIBILITY REQUIREMENTS TO APPLY FOR THIS PROGRAM:

1. U.S. Citizenship is required for all internships carried out at the Texas Department of State Health Services (TDSHS). This is not a requirement for internships carried out at the Austin Travis County Health and Human Services Department (ATCHHSD).

2. Enrollment at the University of Texas at Austin as a degree seeker or post-baccalaureate student.

3. Junior standing or better with a “B” grade point average (GPA) or better, i.e. 3.0 GPA or greater.

4. Prerequisite Coursework: BIO 326R, or BIO 326M with a grade of “B” or better, or BIO 226R and BIO 226T. For laboratory internships, BIO 126L (and sometimes BIO 361L) is required.

PLEASE NOTE: If have not completed (or are not currently enrolled in) the prerequisite coursework but believe that you may be qualified to participate because of other experiences, please e-mail Dr. Leanne Field (field@austin.utexas.edu) to discuss your individual situation.
INSTRUCTIONS FOR COMPLETING THE APPLICATION

Note: Please save your responses to questions in a text file or Word document. Don't try to compose essay responses online. You may lose your work!

If you meet the eligibility requirements for the program, follow the steps below. Questions about your eligibility? Email Dr. Leanne Field at field@austin.utexas.edu.

STEP 1:
Fill out your contact and demographic information below, and click on the agreement to share your information.

STEP 2:
You should already have requested two letters of recommendation.

STEP 3:
Complete all parts of this application form - due by Monday, November 26 at 5pm. Please respond to all questions. It is important when ranking projects to provide comments under the rankings, describing WHY you would like to carry out this research project.

It's recommended that you compose your answers in a separate document before filling out the application form. We cannot guarantee that partially completed application forms can be saved. If you need help with answering any of the questions or completing the application, please contact Dr. Field or Dr. Kneeland for assistance.

STEP 4:
Provide a resume and a copy of your official transcript - due by Monday, November 26, at 5pm

Drop off a resume and an unopened copy of your UT official transcript to Dr. Leanne Field's office in Painter 1.06, OR mail to

STEP 5:
Participate in a group interview on Tuesday, December 11, 2012.
All eligible applicants will be informed of the time and place of a "Meet and Greet" event with the internship mentors.
