Question Title

* 1. This form requests your consent to participate in the study being conducted by the principal investigator affiliated with Baruch College of the City University of New York. The data is being collected for research purposes in order to better understand the characteristics of users of the website. No identifying information (including names or addresses) will be collected. The computer IP address may be collected to ensure that there are no duplicate surveys.
