Welcome and Instructions
Welcome to the High Performance Prosecution Mentor Site application. Thank you for expressing an interest in becoming a High Performance Prosecution Mentor Site.
We have developed this electronic application in the hopes of making this process faster and easier for you. Please complete the following application no later than: 5:00 P.M. EST, August 15, 2014.
Most of the questions are of the check the box variety. Some, however, call for narrative responses. The easiest way to respond to these narrative questions is to separately prepare the response in a Word document and then cut and paste the response text into the application.
You need not complete the entire application in one sitting. Feel free to go through several sessions if you like. You can end any given session by simply closing the surveymonkey.com website. Your existing answers will be saved. To continue again, simply hit the survey link again and you will be taken to the page that you were last at. You can navigate backward and forward within your application by using the navigation keys at the bottom of each page. You have to scroll to the bottom of the page to see these keys. You are free to alter your previous answers until you hit "Done" on the final Thank You page.
The questions are broken into separate sections, including one for each of the four components of the High Performance Prosecution Model, one for your agency contact information, one for office and jurisdiction demographics, and one on your capabilities to host site visits.
