This questionnaire contains items about what your personal perceptions are about being Native American and your feelings around being Native American. The following statements are all opinions. Statement responses will vary from strongly disagree to strongly agree. It is designed to measure a wide range of opinions about what people think it means to be Native American. There are no right or wrong answers. Please answer questions honestly as they are based on your own opinions.

Data collected from this confidential survey will be used for completion of a Honor’s Thesis project in Psychology at San Francisco State University. The information gathered will be used for research on Native American Ethnic Identity. The survey questions will be about what your personal perceptions are about being Native American and your feelings around being Native American. You have been invited to participate because you have identified as Native American or have ancestors who are Native American. You must be 18 years of age or older to participate. There are no risks or benefits to you in participating in this survey. You may choose to participate or not. You may answer only the questions you feel comfortable answering, and you may stop at any time. If you do not wish to participate, you may simply exit out of the survey, with no penalty to yourself. If you do participate, completion of the survey indicates your consent to the above conditions.

All research data will be stored in an encrypted document on a password protected computer.
Please do not put your name on this form. The survey should take approximately 30 minutes to complete.
